Wiki Modifier for not finishing a purchased medication

Does anyone know if there is a modifier for Medicare that could be used when a physician purchases Euflexxa which is injected once a week for 3 weeks and the patient decides that she does not want to do the last injection. The medication was already purchased and we are not able to use it on someone else since these are given in cycles of 3. How do I code???

Is it possible the next time to give the patient a prescription for the drug and have them bring it in with them? I am unaware of any modifier that will get the last dose paid.
 
Look up modifier "JW". It is new and it's to be applied to the HCPCS code of the amount of the wasted drug, not the drug that was administered. You show both amounts on two lines when billing. It is only to be used on single dose vials and single use drug packages. It is not to be used on multiple dose vials. "JW" is a Medicare only modifier.
