Wiki Modifier needed for HCPCS J9040

In dermatology, we use J9040 to treat warts for chemical destruction. Humana is stating that there needs to be a modifier. We have used modifier 59 as patient had other lesions treated, and is still denied. We use dermcoder to confirm the modifiers that are being used and it states not modifier is needed. I am lost on what modifier is needed as it is the medication and not the CPT that is being denied for modifier.

A 59 modifier would not be used on a J code. They may be looking for modifier JZ or JW (JZ to indicate no waste or JW to indicate how much waste there was).
