modifier needed on UGI endoscopies

Anyone in Mississippi do billing/coding for endoscopies on Magnolia Medicaid replacement patients? I have a claim for 43239 (outpatient) by a general surgeon that is denied because of no modifier. It's the only service rendered, nothing unusual about the procedure. No modifier seems appropriate. Anyone have any ideas???
