Can anyone help me better understand modifier Q0? Per the HCPCS book, Investigational clinical service provided in a clinical research study that is in an approved clinical research study. It's confusing me when I would use this. Additionally, do any commercial insurances reimburse for this? Thanks so much.

Approved investigational clinical services consist of services that the provider investigates as an objective within the study. Not all investigational clinical services are approved. Investigational clinical services may include services that are approved, unapproved, or otherwise covered or not covered under Medicare.

Basically, we only ever used Q0 for services we knew were being performed but were not part of the approved trial. Hardly ever happened but we were a free-standing provider based clinic. The modifier might be used more in a facility setting with access to a broader variety of clinical trials.
