Wiki Modifier Question on 63030 and 63046

I have a patient who had hard wear removal 22852 however she returned hospital to a month later to address her back pain
A procedure was done 63030 LT and 63046 Lt
I am having trouble getting the insurance =- HUMAMA MEDICARE RELACEMENT-to pay for procedure.
I had used a 79 but they say not valid modifier.
Needs some help-- I'm trashing on this

You may be getting a response of not a valid modifier due to the LT modifier for 63046. I don't believe a laterality modifier is an option for this code. Also make sure your dx is not the same as the original surgery if you're using the 79 modifier.
