I have conflicting information as to when modifier QW should be applied to CLIA tests.

Should it be applied to?
1) Medicaid plans including the HMO's (Ohio)

2) Commercial Insurances

this modifier is to be reported if your office/testing site is performing a CLIA waived test. Not all tests require this modifier be appended to the CPT code. Hemoglobin A1C tests require a QW. It does not depend on "who" the insurance company is, it depends on whether the test needs the modifier... theres a list online... google "CLIA WAIVED TESTS".
