Wiki Modifier -SA Question

Hello. Hoping to get some clarification regarding modifier -SA. Is this used when a commercial payer does not credential PA/NP or should it be used to identify services performed Incident-To?

Does the carrier require modifier SA? I am not familiar with any carriers that require it.

Effective from May 2017, UHC requires SA modifier on Mid-Level providers.

Also, some of few insurances need SA Modifier to know the service was rendered by NP/PA, Texas BCBS
