My provider billed a 99394, 99213 and a 17000 there is plenty of documentation to support all 3 codes I put a 25 mod on the pex and a 51 mod on the 17000 but the payer (Medicaid) came back and said on the 17000 services are included in the pmt for another service. What am I doing wrong?
 
Modifier

The modifier 25 should be on the OV not the PE visit. This will probably fix your situation.
