I need to know the latest on billing using modifier "SG". We have a facility in the office we use for surgery, the biller is using SG RT (surgery, right eye). BCBS is denying the claim stating modifier is inconsistent or missing with the procedure. She has been billing like this for years and claims have been getting paid. Is this something new?
