Could someone help, please.
I billed office visit 99213 and testing, and got paid for the testing and OV was denied with "The procedure code is inconsistent with the modifier used or a required modifier is missing. Note: Refer to the 835 Healthcare Policy Identification Segment (loop 2110 Service Payment Information REF), if present.

My claim:
99213 - 25
95024

I got many claims with this issue, some claims got paid.

Try putting 95024 as primary.

The RVU for that test is minute. I think the E/M needs documentation to show things other than allergy testing.
