I have a provider who saw a patient in an acute care rehab facility. She saw the same patient again on the same day when family was visiting and examined pt legs again due to family request. Is there a modifier to use so I can bill both visits
 
Hello, i am not an expert but mod 76 is for a procedure, in your case it's e/m. so , you have 2 e/m for the same md, same specialty, same reason, same DOS! Option 1- combine the time for 2 visits and bill one e/m based on total time. Option2- if for Another unrelated reason, then i read that you can use mod 25 ( i recall) or it's unbillable.
 
Combine total time from both visits....

mod 25 is only applicable if it is a separately identifiable e/m (no overlap in work). You should not be appending it to bill the same service 2x (which is essentially what you would be doing)
