Check out Ingenix Understanding Modifiers. It will teach you the proper use of modifiers for surgery, E/M, radiology, etc. There are examples which is really helpful. A little pricey around $100, but a great investment. Amazon or ebay may have used copies for cheaper.

There is a great book written by Deb Grider called Coding with Modifiers. It is published by the AMA. I find it to be a great reference.
