I have a denial for missing modifier for E&M 99223, where modifier 57 was applied for decision of surgery in the same day. The provider did a wound debridement along with a device implant and my E&M code was denied for missing mod 25. can both modifiers be applied for the E&M code? 57 and 25? I am confused. Any advices?
The billing: 99223/57 and a separate claim for 33262 and 11042/59. Medicare is looking for modifier 25 for CPT 11042.??
