I have a case where we sent a claim to the ins where the pt had to have a urine culture 87086 sent due to dysuria 788.1 and the dr also sent a vaginal culture 87070 due to vaginitis 616.10. The ins is requesting a modifier for the vag culture 87070 but I'm unsure if the appropriate modifer to use on one of the codes is the 91 modifer. :confused:
