Wiki Modifiers and add on codes

I have been a certified coder for a long time and have always been told and applied that modifiers are not to be used on an add on code. I am unable to find any documentation of this now, did it change along the way? I am just too old haha.

Please help
 
The Introduction in the CPT Manual discusses Add-on Codes and 51 modifier (AMA 2019 edition, page xv). Sorry but I don't have it available to copy/paste. I'm not aware of it being documented anywhere else so I'll be curious to see other replies to this thread.

It has always been my understanding that as add-on codes, they should not require 51 or 59 modifiers. However, PAYER guidelines are different from CODING guidelines and certain plans bundle some add-on codes unless they are reported with 59 modifier. I work in Oncology and find that 96411, 96367 and 96375 regularly bundle if we don't use a 59 modifier.
