Are there any urology practices using these two modifiers? If so, on what medications?

In urogyn, we use on:
J0585 onabotulinumtoxina 1unit (Botox) used for chemodenervation of bladder
J1212 dimethyl sulfoxide 50 pct 50ml (dmso) used for bladder instillation
There may be others used by your practice. It is basically used for any J-code that is in a single use vial.

These modifiers are for single use vial drugs - which can be verified by the NDC, and the drug also is separately payable. Bundled drugs do not require modifier JZ - this can be verified using the CMS OPPS

This is typically a problem with Botox claims.
