Wiki Modifiers needed when there is a surgery assistant.

We have an APRN that will start assisting our MD during surgery. From what I am reading I would use the AS modifier since she is a non-physician. I wanted to know do I report the surgery on two separate claims and append the AS to the APRN's claim and if so what, if anything, do I append to the MD's claim? Or how would I report this to the payer so that they know that the MD performed the surgery and the APRN assisted during the surgery?
 
It should be two seperate claims. Nothing is needed on primary surgeon claim, they will know he/she is primary because there is no modifiers. We have a PA, so on her claims I add the AS modifier.
