Wiki Modifier's with Physical Therapy and Physical Therapy Assistant

I work for a Family Practice, but we have a Physical Therapy Dept as well. We just started using a PTA in the office. I am having issues with Humana stating the incorrect modifier is being used. The only things I have read about is using GP for the Physical Therapist and CQ for the PTA.

Has anyone else had this issue and have you found any documentation regarding this?
