Wiki Modifiers

I recently billed to BC/BS of NJ cpt code 38230-59LT and 38241-59LT. They were both denied stating procedure code is inconsistent with modifier used or a required modifier is missing. Can someone help me?

What is your rationale for using the 59 modifier on both of these codes.
