When performing a chart audit, if the physician documents "Any Medications given- none" in the HPI would this still count as a point for the Modifying factor?
 
I believe that would be considered a modifying factor note. I often write something similar to that for my patients who have eye irritations or infections and haven't tried any "home" treatment prior to seeing me when I note the HPI.
