MY doctor would like to know if anyone is getting denied for MOHS from Universal, stating denial as not medically necessary because of location (site); diagnosis codes of Basal cell carcinoma, Squamous cell carcinoma. Pre-auths are obtained and later denied. We have reduced the number of MOHS to sites only on the head, neck. We do excisions primarily on the body. Has anyone experienced this kind of scrutiny from Universal?
