Has anyone billed a Mohs procedure with lower eyelid repair using lower eyelid tarsus reconstruction and a rotation flap? I think my provider is wanting to bill 17311, 17312, and 67961 but these are mutually exclusive procedures according to NCCI edits. Also I need clarification on his documentation. Has anyone had anything similar to this scenario before? Can anyone give advice on billing these procedures? Thank you.

These are mutually exclusive because 17311 and 67961 are both excision codes - you wouldn't code a separate eyelid excision since the Mohs procedure already encompasses this. A repair/reconstruction of the eyelid via a rotation flap would usually be coded as an adjacent tissue transfer with CPT code 14060 or 14061.
