Wiki MOHs with permanent section 2nd layer

Dermatologist performs a 2 layer MOHs but sends the second specimen for a permanent section and consult by the pathologist. To qualify for a MOHs code, the doc has to be both the surgeon and the pathologist. I've never had this scenario happen in 16 years of coding these. Any ideas?
 
for his portion I believe the pathologist would code 80500 or 80502 depending on if records were sent to him with the specimen or not.
 
I was wondering how the surgeon bills for the second layer of a MOHs without reading the frozen section slides herself.
