My providers use the below as a status and now im wondering if it can be considered a status and counted toward MDM. Patient didn't come in for cataracts and its not mentioned in the HPI. Exam states: 1-2+ Nuclear Sclerotic Cataract

3. Nuclear sclerotic cataract of both eyes
NVS ou., monitor

Is this enough to go off of?
