What code is everyone using for an arthroplasty w/implant of 1st MPJ foot when no bunion is present???

I have been using unlisted, all of a sudden I am having increasing difficulty in getting this paid, with commercial ins. even had one denied as "experimental"

Any input would be appreciated.
