I need some guidance...We had a claim denied with that had MRA codes attached. But I want to know if we submitted it correctly. It's a Medicare patient and we billed the G0439, 99212, 99499 in order to capture all the diagnosis/MRA for the patient. Of course it's the second half of the year and our doctors are capturing the conditions early. So I just want to know if we billed it correctly? We got a denial on the 99499 so I just want to know ow others would bill a patient with 16+ diagnosis and what CPT codes would they use. What CPT codes would you use on each set of DX codes? Please advise....thank you in advance.
