If a mediastinoscopy is done on one day and b/c of the mediastinoscopy it is decided to do a mediasternotomy with removal of masson another day how would you code/ ie: what modifiers,

If it is within the 10 day global period of the mediastinoscopy, I would add modifier -58. This modifier is used for planned/staged procedures or if the subsequent procedure is more extensive than the original (but still related).

If unrelated to the first surgery, I would add -79.
