I have a situation where the physician has removed two malignant lesions on the patient's face. He then performed two separate transpositional flaps to repair. I realize that the excisions are not seperately coded, but are the repairs coded seperately or are they added together since they are both on the same area of the body? Would I code 14041 once or twice with 59 modifier? Anyone have any experience in this area?
 
you code the excisions separately if they were excised separately, if they were two lesions excised as one excision then you have only one excision code , you will code both flaps as well if they were two different flaps covering two different areas. Can you give more info from the op note?
