Claim denied by United Healthcare for multiple immunizations/administration. Series of immunizations and administations all paid but the last administration.
Reason of denial was duplicate. Appealed and appeal was denied. Called insurance and was told we need a "Maximum Frequency Modifier. "
Never heard of that - does anyone know what modifier that is. Unable to find anything when researching it.
