What do you think the best coding scenerio is for the following:

LHC &
Angioplasty and stenting in all of the following:
LAD
Ostial Proximal portion
Proximal Left Circumflex
Mid Poriton Left Circumflex

Medicare PART B provider..

93458-26/59 (cath), 92928 (LD) & 92928-59 (LC)
The stent placements in the "mid" portions ext. are still the main vessel and there for no additional codes are added. Angioplasty done in these main vessels are also bundled.
