I have a Dr who did a left heart cath, FFR, PTCA of LAD-no stent, then Stent of RI, Stent of LC, and a stent in the posterior descending. I could really use some help coding it as I have never had a Op Note like this before.
 
First is you code by what vessel was stented, not by how many stent were placed. So I would code this as 93458-59, 92920-LD, 92928-RI, 92928-LC, 92928-RC. If you are coding for the hospital, and drug-eluting stents were placed, change 92928 to C9600.
