Is the any hard and stead fast rule that insurance always have to apply the multiple
surgery rule or can this be appealed?

The multiple surgery rule is pretty much universally applied ... and with good reason.

After all you don't have double the pre- and post-operative care just because you performed two surgeries in one session. The reduction for the subsequent surgery takes this into account.
