MRI shoulder has been done. Findings are supraspinatous tendon, subscapularis and infraspinatous tendon tear. What should be coded? Dx code 840.8 alone or 840.X series codes for each site.

I would code ''840.6 Supraspinatus (muscle) (tendon) sprain and strain'' and ''840.3 Infraspinatus (muscle) (tendon) sprain and strain''
because both having specified diagnosis code if ''840.8 Sprain and strain of other specified sites of shoulder and upper arm'' is other specified site of shoulder
