The patient had multiple toenail avulsions on both feet. We aren't quite sure on the billing for this. Does this look right?
11730- TA
11732- T1
11732- T2
11732- T3
11732- T4

11732 is an add-on code so you wouldn't list it multiple times nor would you use the anatomical modifiers. The primary code 11730 has an MUE of 1 and +11732 has an MUE of 9, so the codes are already set up to count individual nail plates up to 10. If 5 are removed, then you'd have 11730, 11732 x 4. If all 10 toes had nail plate avulsions, then it'd be 11730, 11732 x 9.

The only time you'd run into problems is if you tried billing for more than 10.
