My question: Our physicians do not always remember to add the order for 93015 when entering orders for myocardial perfusion scans 78452[stress/rest] and 78451[stress only]. Is an physician signed order required for 93015 if he has directed exercise or pharmacological within the order for the myocardial perfusion scan?
