Can someone please tell me if you can code 93571 with a 93458 and a 93454? We have a Cardiologist insisting that we can bill the 93454 along with the 93458 for the FFR.
 
You cannot bill 93454 and 93458 -- that would be considered unbunding. 93458 includes the 93454. You can however bill the 93458 or 93454 with the FFR 93571.
