Can anyone help with the following:
Possible melanoma of nail matrix and nail bed.
A nail spatula was used to pry the undersurface of the nail plate from the nail bed as well as the nail folds away from the nail plate. The entire nail was removed and submitted to pathology. Two punch biopsies were performed, one of the right proximal nail matrix and one of the right distal nail matrix. A linear wedge incision was then perfomred to remove the right lateral nail bed and the right lateral nail matrix. This was performed from the distal edge of the finger to the proximal matrix.
What is the proper way to code this?
