Does anyone have a good resource for how to complete the additional information needed on a claim for the National Drug Code? Which NDC code do you use (package, vial, CMS crosswalk)? Determining the number of units; billing for the waste; etc.?

On the CMS website you can find information in the drug pricing files, and NDC crosswalk. It pulls an excel spreadsheet that shows the NDC number, units, etc. I always use the NDC on the actual vial.

The NDC number is on the medication packaging. How it's reported regarding dosage can be found in the HCPCS manual, as it's in the code description. (For example, J2704, Injection, propofol, 10 mg)
