Is T88.7XXA, T45.1X5, R11.2 correct for nausea and vomiting after chemotherapy?
 
I don't think you need to code T88.7XXA since the condition isn't unspecified, its given as nausea and vomiting. Since its adverse effect you will have to give the nature of adverse effect as pdx.
 
Coding Nausea and Vomiting from Chemotherapy

It is a R112. Nausea with vomiting.
Also a T451X5A- Adverse reaction, initial encounter.

Nausea and Vomiting is a side-effect of most chemo. I would not code these as an adverse reaction.
