Hi. So I am wondering what the special formula is for getting NC Medicaid to pay for fracture care in the ER? Put the 57 modifier on the EM, the 54 on the CPT and included RT or LT etc. Am I putting too many things on the claim? What am I doing wrong? Thank you for any advice you can give! :o
 
If documentation supports. The 57 modifier would go on the E/M and the 54 on the CPT and the anatomical modifiers as well. I don't think you are putting too many things on the claim. IF this is all supported through documentation.
