Can anyone share with me how to calculate the NDC dosage of a drug?

I know that it should be coded in ML but the J code unit is MG.
EXAMPLE: J9299 Opdivo 1 mg. Patient given 240 mg out of two different Vials; NDC#00003-3774-12 100mg/10ML and NDC# 00003-3772-11 40mg/4ml.

Thanks for any help with this.
 
HCPCS is per mg.

NDC indicates mg per ml so you need to know how many ml were injected to convert to mg

240mg would be

2x NDC#00003-3774-12 100mg/10mL
1x NDC# 00003-3772-11 40mg/4mL.

If this is what was dispensed then you would bill 240 units of J9299
 
NDC dosage

Yes I know that we would bill 240 Mg of J9299 but I need to know how many mL to bill for the NDC dosage. Yes we gave 2 vials of the 100mg/10mL and 1 vial of the 40mg/4mL, but I don't think billing 2 and 1 for the dosage would be correct. I need to know if there is a formula for calculating MG to ML and if so, where I might find it.

Its 24mL total between the 3 vials administered. You add up the mL as specified by each NDC

NDC#00003-3774-12 100mg/10ML. This is a 10mL vial so 2 vials would be 20mL.
There is a total of 100mg in a vial

NDC# 00003-3772-11 40mg/4ml. This is a 4mL vial so 1 vial would be 4mL.
There is a total of 40 mg in a vial

You don't need to calculate anything unless a partial vial is used.
