Code A7003 was billed on a claim for a nebulizer done in office along with 99213, 94640, and j7613. The A code was denied and I can't seem to find information from the payer. I asked customer service if it needed an NU modifier but was told that it wouldn't make a difference. Does anyone know if I am missing a modifier here? The code was not denied as content of another service, it was denied as the service not being "allowed." The patients plan and our contract do not exclude it specifically.
