A weighted speculum was placed in the vagina and a single tooth tenaculum was used to grasp the anterior lip of the cervix. At this time, a large endometrial polyp was noted to be filling the vagina. This was grasped with a tenaculum, twisted and removed in its entirety. The specimen was sent to Pathology. The hysteroscope was then inserted into the cervix with the use of normal saline as a distending medium. Normal endometrium was visualized and bilateral tubal ostia were also visualized. The hysteroscope was removed. The NovaSure device was inserted into the uterine cavity and set to a cavity length of 6 cm and width of 3.6 cm. The device cycled through its initial assessment and was unable to achieve a proper vacuum. This was tried multiple times with placing an Allis clamp on the cervix to help reduce the diameter at each time. I was never able to achieve a proper vacuum. The procedure was aborted at this time. All instruments were removed from the uterus, cervix and vagina. The patient did not receive antibiotics prior to start of the procedure. The sponge and instrument counts were correct x2. The patient tolerated the procedure well and was brought to the recovery room in a stable condition. shall i code 58558, 58563- 53
