Procedure​

Informed consent: Yes. After informed consent was obtained and patient pregnancy (females only) was ruled out, an intravenous line was established in the right hand. Pharmacological stress testing was performed with intravenous administration of 47.9 mg of Lexiscan over a 04:00 min infusion. Maximum predicted heart rate: 159 bpm. Target heart rate: 135 bpm. The heart rate was 62 bpm at baseline and rose to 69 bpm, which corresponds with 44% of the maximum predicted heart rate. Resting blood pressure was 130/84 mmHg and peak effect blood pressure was 132/84 mmHg. The blood pressure response during exercise was normal. During stress the patient experienced symptoms which included fatigue . Stress symptoms resolved during recovery . The stress test was terminated because of fatigue.

Myocardial Perfusion Imaging Procedure​

The patient had myocardial perfusion imaging performed using a rest Tc-99m/stress Tc-99m one day. Ungated tomographic myocardial perfusion imaging was performed at rest 45:00 min following the intravenous injection of 11.1 mCi of Tc99m Sestamibi. Al peak pharmacological effect, the patient was injected intravenously with 30.2 mCi of Tc99m Sestamibi. Gated post-stress tomographic imaging was performed 45:00 min after the stress isotope injection.

Rest and Stress ECG​

Normal Sinus Rhythm. Non-specific ST abnormality during rest. Stress ecg shows no changes from baseline.

Findings​

There are moderately reduced perfusion defects of medium size in the basal, mid inferolateral wall. The defect in the inferolateral seqment is oartiallv reversible. The left ventricular chamber dimensions are normal. Normal wall motion. Pharmacologic stress nuclear study is abnormal.

Impressions​

There is medium scar of the inferolateral segment with mild residual ischemia. 3 . Normal left ventricular systolic function .
