How to code Lesion Excision?

Physician has performed excision of 5 scalp lesions 4cm each and also closed with layered closer. How to report the lesion excision? Should I need to give 11424 5 times or need to all lesions size 4cm*5=20cm, 11426 only once? Please help it out.

excisions are all coded separately do not add together , do not report once with units, report a separate code for each one with a 59 modifier on the second and subsequent code. For the repair you add the lengths together and code one repair code. Since these are excisions you must wait for a path report befor coding to know if you need a malignant excision code or benign, and to know the dx code.
