I am billing for the ASC. Where can I find a list of codes allowed to be billed in an ASC per Medicare?

Of interest is code 67028 with drug code J2778, J0178, J9035.

When you open the code and payment rate files from the CMS site, you'll have a number of different addenda. Addendum AA is the list of covered surgical codes; addendum BB has the list of ancillary services that are either integral to procedure (payment indicator N1) or separately reimbursed (Z2 or Z3). Addendum DD1 will give you a list of the different payment indicators and what they mean.
