i have a patient that saw our obgyn provider for her annual exam. coded as an annual. 6 months later patient went to see her regular provider and in the provider documentation they put yearly/establishing relationship. in this documentation it doesn't state annual or general nor did the provider put that code in the diagnosis end of the documentation. the coder for this provider changed the provider's regular office visit/e&m code and put this as an annual as well. the obgyn provider's documentation did reference that visit as an annual visit. the patient's insurance only pays for one yearly per year. my question is, is the obgyn visit the annual or does the regular provider visit get the annual??

I have seen this go two different ways. GYN codes with diagnosis of Z01.411 the PCP codes with diagnosis of Z00.00. Since the PCP office coded as routine exam, curious to what diagnosis they used?
