THIS IS THE OP NOTE:

PREPROCEDURE DIAGNOSES: Carotid artery disease and coronary artery disease
with positive stress test.

POSTPROCEDURE DIAGNOSES: Obstructive 3-vessel coronary artery disease and
obstructive left carotid disease.

OPERATION/PROCEDURE: Coronary angiogram and aortic arch angiogram, right
carotid angiogram, left carotid angiogram, IVUS of the LAD and PTCI of the
LAD.

PROCEDURE PERFORMED BY: M.D.

ANESTHESIA: IV Versed and fentanyl with local lidocaine.

COMPLICATIONS: Mid LAD dissection. FINDINGS:
1. Heavily calcified mid left anterior descending with a 95% lesion in an
otherwise small caliber vessel.
2. A 100% proximal right coronary artery with collaterals.
3. Diffusely atretic circumflex with 100% obtuse marginal vessel.
4. Obstructive left carotid disease with a 75% left internal carotid artery
and a 75% left external carotid artery lesion.
5. Dissection in the mid left anterior descending treated with 1 stent with a
residual controlled dissection with no propagation in the mid left anterior
descending.
