I am new to Pain Management and I am hoping someone can shed some light on this for me. The Procedure performed was a Lumbar Transforaminal Epidural Steroid Injection with Fluoroscopy, right side, Level L4 and L5.

Would this be coded as follows:

64483-RT
64484-RT

What is the difference between single level and single joint?

A transforaminal epidural targets the nerve root that exits between two vertebrae. You should read your provider's dictation carefully to determine how many nerves (levels) they are blocking. Some physicians may say the nerve at L4-L5, and some may say the nerve at L5. Most of my providers document a single nerve as L4-L5, ie the vertebra above and the one below the nerve being treated. A single joint is also defined as two vertebrae.

If your provider is actually doing L4-5 and L5-S1 then your code choice is correct. If he/she is only injecting the nerve between L4 and L5 on the right you would code as

64483-RT
