Need help: E/M coding with othotics & inserts

When a patient is fitted for shoes, orthotics or inserts, does the E/M get billed? To me, it makes sense that it does. BUT...what about when delivery of shoes or orthotics is made to the patient...is an E/M billed again or is that still included as a part of the fitting of the units?
 
We bill for the initial fitting or casting, but when they are picked up we bill a 99499 unlisted E/M with no charge and noted as a ortho pick up
